Service Description
Choose this service when an employer, laboratory, TPA, court, provider, or other organization has already arranged your testing and you only need professional urine specimen collection. Please bring your testing authorization, chain-of-custody paperwork, electronic passport, or laboratory instructions when applicable.
Cancellation Policy
Appointments must be canceled or rescheduled at least 24 hours before the scheduled appointment time. Late cancellations, missed appointments, and no-shows may require a new booking and may be subject to applicable fees. Mobile service and after-hours appointments may have additional requirements or fees. Please arrive on time with a valid photo ID and any required testing documents, authorization forms, or laboratory paperwork.
