Appointments may be scheduled online, by phone, or through an authorized representative.
To help us provide timely service, clients should:

Mobile Services

Employer & Corporate Accounts
Businesses, employers, healthcare organizations, and Third-Party Administrators (TPAs) may establish billing accounts with Smith Lab Testing LLC upon approval.
Corporate accounts are subject to separate service agreements, billing terms, and payment schedules.
Laboratory Services
Smith Lab Testing LLC provides professional specimen collection services.
Laboratory analysis is performed by accredited partner laboratories when applicable.
Laboratory turnaround times, testing methods, and final reports are determined by the performing laboratory.
Smith Lab Testing LLC cannot guarantee laboratory processing times or testing outcomes.
DNA Testing Services
Smith Lab Testing LLC collects DNA specimens for submission to accredited laboratory partners.
Collection services do not include interpretation of laboratory results.
Clients are responsible for selecting the appropriate DNA testing option before specimen collection.
Legal DNA testing requires strict chain-of-custody procedures and valid identification for all participants.
