Service Description
Smith Lab Testing LLC provides professional Notary Public services for individuals, families, and businesses. Schedule an appointment for acknowledgments, jurats/sworn statements, oaths or affirmations, signature witnessing, and other eligible documents. Mobile notary service is also available for approved locations. Notarial fees, mobile travel fees, and applicable after-hours charges are calculated based on the services requested. Please bring valid government-issued identification and the documents requiring notarization to your appointment.
Cancellation Policy
Appointments must be canceled or rescheduled at least 24 hours before the scheduled appointment time. Late cancellations, missed appointments, and no-shows may require a new booking and may be subject to applicable fees. Mobile service and after-hours appointments may have additional requirements or fees. Please arrive on time with a valid photo ID and any required testing documents, authorization forms, or laboratory paperwork.
